H1N1 swine flu still around, CDC says

Source: (Reuters) Thu Apr 15, 3:13 pm ET

WASHINGTON (Reuters) – H1N1 swine flu continues to cause epidemics, especially in the southeast of the United States, U.S. federal researchers reported on Thursday.

Americans who have not been vaccinated should still try to get the shot or nasal spray, the team at the U.S. Centers for Disease Control and Prevention said.

The CDC’s weekly surveillance report on influenza mirrors the World Health Organization’s warning that the pandemic continues, although at low levels. As long as the virus is being passed among people, it could change and re-emerge at any time, infectious disease specialists say.

“For the week ending March 27, 2010, pneumonia or influenza was reported as an underlying or contributing cause of death for 7.9 percent of all deaths” in the United States, the CDC said. This is slightly above the epidemic threshold of 7.8 percent that the CDC has set for the last week of March.

More than half the reported cases were in Alabama, Florida, Georgia, Kentucky, Mississippi, North Carolina, South Carolina, and Tennessee.

Children were the hardest-hit, in contrast to seasonal influenza, which is usually worst among the elderly.

“This season’s cumulative hospitalization rates from August 30, 2009, through the week ending March 27, 2010, remain highest in children aged 0-4 years,” the CDC said.

Earlier this week John Mackenzie of the WHO’s Emergency Committee said the H1N1 flu pandemic is as severe as influenza pandemics in 1957 and 1968 — which killed 3 million people between them.

The CDC estimates that H1N1 has killed 12,000 Americans and put more than 265,000 in hospital.

Between 81 and 91 million doses of influenza vaccine have been administered of the 162 million doses shipped across the country, the center said.

Early in the pandemic, there were long lines of people clamoring for H1N1 vaccines but there was not enough to go around. By the time vaccines were available in ample supplies, most of the public had lost interest.

The United States has contracts with five influenza vaccine makers — Novartis, AstraZeneca unit MedImmune, Sanofi Aventis, GlaxoSmithKline and Australian vaccine maker CSL.

The doses remain safe, and people who have already been inoculated don’t need to get vaccinated again, said Norman Baylor, director of vaccines research and review for the U.S. Food and Drug Administration. About 3,000 of the affected doses remain in warehouses, and AstraZeneca will send recall notices to clinics that may have unused vials of the vaccine.

The recall follows a similar move on Dec. 15 by Paris-based Sanofi-Aventis SA, which recalled 800,000 swine flu shots. Manufacturers maintain samples for testing from each vaccine lot that is shipped, and they continue to test them for waning potency, Baylor said. Both recalls were made voluntarily after the strength dropped below pre-specified thresholds.

“There are no safety concerns with these lots,” Baylor said in a conference call with reporters yesterday. “We do see a decline in potency every now and then.”

The doses were distributed in October and early November, and the “vast majority” have already been administered, said Tor Constantino, a spokesman for London-based AstraZeneca, in a telephone interview yesterday. The doses passed all potency tests before the vaccines were shipped. The company has never seen a similar potency decline with its seasonal-flu version of the FluMist nasal spray vaccine, he said.

60 Million Vaccinated

At least 60 million people in the U.S. have received one of several kinds of swine flu vaccine, out of 111 million doses available as of this week, the U.S. Centers for Disease Control and Prevention reported yesterday. After initially limited supplies, most states are now offering free inoculations to anyone who wants one, said Anne Schuchat, head of the CDC’s National Center for Immunization and Respiratory Diseases, on a conference call.

AstraZeneca has shipped 23 million doses out of its contract for 40 million with the U.S. Health and Human Services Department, Constantino said. The U.S. ordered 251 million doses of swine flu vaccine from five companies, and most states now have enough to offer the vaccine to people of all ages. The shots and nasal sprays are paid for by the U.S. government, though clinics can charge a fee to administer them.

Vaccine availability across the country has been rising, even as illnesses are on the wane, according to the CDC. Swine flu rates have fallen for seven straight weeks and are at their lowest level since pupils returned to classes in September.

Clinging Antigen

Sanofi’s decreased vaccine potency may have been due to the key ingredient, antigen, clinging to the wall of the syringe over time, the CDC’s Schuchat said after the recall.

AstraZeneca’s vaccine has a different type of applicator and is made using a different process, the FDA’s Baylor said. Scientists are still trying to determine what was responsible for the reduced potency. Such decreases are rare but not unheard of, he said. A batch of seasonal flu vaccine was recalled for the same reason in February, FDA spokeswoman Karen Riley said in yesterday’s conference call.

H1N1 infected 50 million people in the U.S. and killed an estimated 10,000 through Nov. 14 from the start of the pandemic in April, according to CDC estimates reported Dec. 10. About 36,000 people die each year in the U.S. from influenza.

Most swine flu infections are no worse than seasonal flu, according to CDC officials. In rare cases the disease leads to severe illness and death in otherwise health people. Unlike seasonal flu, swine flu disproportionately attacks the young. In a typical flu season, 90 percent of deaths are over age 65; with swine flu, the effects are reversed, with 90 percent of deaths under age 65.

In recent weeks, humans have not been the only mammals to contract H1N1. Several cats and ferrets have contracted the virus proving that animals are susceptible to swine flu as well.
“The influenza virus has molecules on its surface that allow it to attach to host cells. Many species of animals have influenza, but usually a particular strain of the influenza virus is restricted to infect only its specific host,” Laura Vogel, associate professor of immunity, said.
“For example, we know that there is a canine influenza virus that can make dogs sick and spread from dog to dog, but people can’t get that strain of the virus.”

The H1N1 virus, however, has mutated so that it is able to attach to host cells of other animals and humans, thus infecting them with the virus.

“The problem is that influenza viruses can change their surface molecules, and when this happens, which is rare, the virus can infect a new type of host. That is what happened in the case of H1N1, which seems to have properties of influenza viruses that would normally infect only birds, only pigs or only people. This new mutated virus then can infect more host species.”

H. Tak Cheung, director of the school of biological sciences, explained the virus is much like a key and lock phenomenon.

“The virus must have the right key to open the right lock in order to get entry to the cells. The key that would unlock human or pig cells would not unlock the other type. However, imagine the key makers might make a tiny little error in making the key, and this is the result of mutation, a change in the genetic material of the virus…this slight change will allow, normally, a virus for a pig to fit humans. Now this virus crossed the species barrier.”

Humans are also able to pass the flu to animals through the same methods.

“Of course the same thing can go in the opposite direction, a virus normally for humans will be able to infect a pig, because of this change in the protein through mutation… The lock can also change, allowing it to fit the key,” he said.

“Primate cells are very similar to our cells and most human influenza viruses can infect other primates. This is a rare event, but again not surprising since the virus can change its surface molecules,” Vogel said.

“The reason people worry is because it normally infects pigs, because it infects pigs, it has structures slightly different from a virus that would normally infect the human population,” Cheung said.

“If this virus crosses the species barrier, it might cause more serious infections because the population might not have immunity against this specific virus because it’s new and many people will be susceptible to it,” he explained.

“The influenza virus has a very, very high rate of mutation and this allows it to change rapidly… This change is responsible for the continuing threat of the influenza virus every season,” he continued.

“Normally the virus only changes slightly from one season to another when it is a human influenza virus, but then when you have a virus that jumps the species barrier, the population becomes far more vulnerable because it comes from another animal. This incidence of jumping across the species barrier does not occur frequently, but when it does occur, it creates a much greater threat as compared to the influenza viral strain that occurs in humans from one season to another.”

Vogel explained there are many diseases that spread between animals and people. These are referred to as zoonotic diseases and include familiar diseases such as rabies and West Nile virus.

“At this time, there aren’t any reports of people catching the H1N1 from their pets and you are much more likely to catch it from an infected person,” she said.

“Don’t forget this virus is easily killed by basic sanitation methods like soap and disinfectants. It is killed at normal cooking temperatures and not very stable on surfaces, so it is primarily spread through respiratory droplets. The study of zoonotic diseases has become increasingly important with the emergence of new diseases like H1N1, West Nile and ‘bird flu.’”

The ministry stressed that, as a ratio of its population of 190 million, Brazil’s mortality rate from the virus ranked 7th in the world.

Argentina, Chile, Costa Rica, Uruguay, Australia and Paraguay all had higher rates on that basis, it said, referring to data from the European Center for Disease Prevention and Control.

The United States, which has a population of 300 million, came 13th on the list.

The Americas is the worst-hit region in the world in terms of swine flu deaths, accounting for more than 90 percent of the global count given by the World Health Organization.

After Brazil and the United States, Argentina is the country to suffer the most, with at least 439 swine flu deaths.

http://www.yonhapnews.co.kr/bulletin/2009/07/20/0200000000AKR20090720033000017.HTML?did=1179m
