				<content:encoded><![CDATA[<p>중국에서 발생한 신종 플루 A(H7N9)에 대한 ECDC의 위험평가<BR><BR>3월 31일 중국 보건당국은&nbsp; 신종 조류인플루엔자 바이러스 A(H7N9)에 3명이 감염되어 질병이<BR>발생했으며, 2명이 사망했따고 발표했음. 감염자에게서 A(H7N9)&nbsp;및 A(H9N2) viruses를 검출하였음. (이것은 신종 조류인플루엔자 A 바이러스라는 사실을 의미함)<BR><BR>발병 사례들 간의 역학적 연관 관계는 전혀 나타나지 않았음. 바이러스에 감염된 상하이의 젊은 남성이 도축장에서 일하였음에도 불구하고 해당 인플루엔자 바이러스에 감염되어 사망한 조류 및 동물은 전혀 보고되지 않았음.<BR><BR>신종플루 바이러스의 haemagglutinin&nbsp;및 neuraminidase&nbsp;유전자는&nbsp;A(H7N9)에서 유래한 <BR>재조합 인플루엔자A 바이러스이며, A(H9N2)에서 유래한 다른 6개의 유전자 조각이 섞였음.<BR>(이것은 기존 유럽에서 발생한&nbsp;&nbsp;A(H7N9)와 차이가 있음) 조류의 고병원성과 연관된 유전적 마커는 없는 것으로 밝혀졌음.(추가적인 실험실 검사가 필요함)<BR><BR>M 유전자에서 adamantane&nbsp; 저항성이 있는&nbsp;마커인 S31N가 존재하므로 이 바이러스는<BR>adamantanes (amantadine and rimantadine)에 대한 저항성(내성)이 있을 것으로 추정됨.<BR>따라서 이 제제에 대한 약물투여는 소용없음. 비록 H3N2&nbsp;바이러스에서oseltamivir와 zanamivir<BR>의 저항성&nbsp;(내성)을 갖는 neuraminidase의&nbsp; R292K 아미노산에 대해&nbsp;현재 세계보건기구 협력<BR>연구소에서 phenotypic test를 사용한 확정 검사를 진행 중에 있지만, neuraminidase 저해제<BR>구성성분에 대한 감수성 있는 것으로 결과가 나올 것으로 예상됨.<BR><BR>표 1. 신종플루 A(H7N9) 확정진단 정보 (3명의 사례)<BR><BR>역학적 특징<BR><BR>현재 중국의 조사로 바이러스가 유래한 동물이나 전염 경로, 인간대 인간 전염 가능성, 잠복기, <BR>감염 위험을 평가하기엔 너무 이른 상황임.&nbsp;불현성 감염(보균자) 및 인간 전염 경로는 중국<BR>보건당국에서 조사가 진행 중이며, 아직 결과가 나오지 않았음.<BR><BR>대유행 위험<BR><BR>현재 어떤 의미있는 방식으로도 대유행 위험이 결정된 바 없음. 유럽의 조류에서 A(H7N9)는<BR>낮은 병원성을 보였지만, 병원성이 낮은 바이러스에 대한 공식적인 조사가 이루어진 바가<BR>없으므로 그 분포에 대한 정보가 불확실함.<BR><BR>과거 네덜란드에서 고병원성 A(H7N7)에 감염되어 사망한 사례가 1건 발생한 바 있으며,<BR>당시 H7 바이러스의&nbsp;PB2의 구성성분 중 E627K와 관련이 있었음.<BR><BR>A(H7N9)-A(H9N2) 재조합 바이러스 및 동물에서 A(H9N2) 바이러스에 대한 실험실 연구결과<BR>는 대유행 가능성이 있다고 나온 바 있으므로 A(H7) 바이러스의 인간 전염 가능성에 대한<BR>우려는 합리적 타당성이 있음.<BR>&nbsp;<BR>그러나 신종플루 바이러스의 대유행에 대한&nbsp;결정은 WHO,&nbsp;미국 CDC, ECDC 등의 국제기구의<BR>감시 및 연구가 필요함.<BR><BR><BR><br />

<P>On 31 March 2013, the Chinese authorities announced the identification of a novel influenza A virus, A(H7H9), in three seriously ill people from two provinces presenting with respiratory infections. So far, no epidemiological link has been identified between those three patients.</P><br />
<P>When testing for the influenza virus, the Chinese Center for Disease Control and Prevention identified genes from both A(H7N9) and A(H9N2) viruses, thus indicating a novel reassortant avian influenza A virus. <BR><BR>In its initial assessment of the situation, the ECDC rapid risk assessment concludes that the risk of the spread of the virus in Europe can be considered low at this stage. However, individual patients coming from China cannot be ruled out. It is thus important to further investigate the source of infection and the mode of transmission to develop ECDC’s assessment further.</P><br />
<P>This event stresses the importance of considering the possibility of zoonotic influenza due to novel influenza A viruses in persons presenting with severe acute respiratory disease. Especially when patients have recently been in countries where animal influenza A viruses circulate and these viruses have recently caused severe respiratory disease in humans. This includes China.</P><br />
<P>ECDC is working closely with WHO and the European Commission and will continue to monitor the situation.</P><br />

<P>미국 질병관리본부(CDC)의 추정에 따르면, 해마다 2만4천명이 계절성 독감으로 사망하고 <BR>있습니다. 지난 2003년 겨울에는 무려 4만8천명이 계절성 독감으로 사망한 것으로<BR>추정하고 있습니다.<BR><BR>올해는 미국과 유럽에서 H3N2 타입의 계절성 독감 바이러스가 조금 더 일찍 유행하고 <BR>있으며, 미국 전역에서 2000여 명이 독감으로 입원했으며, 그중에서 어린이 18명이 <BR>사망했다고 밝혔습니다.<BR><BR>미국은 4주전부터 최근까지 2.8%에서 5.6%로 증가하였으며, 중국 북부지역도 3.2%에서 <BR>4.2%로 증가 추세에 있습니다.&nbsp;우리나라에서 주로 유행하고 있는 계절성 인플루엔자&nbsp;<BR>바이러스는 H1N1형이고, 미국과 중국에서 주로 유행하고 있는 바이러스는 H3N2형입니다.<BR><BR>미국이나 중국에서 들어오는 선박이나 배를 통한 여행객과 화물 등을 통해 그 지역에서<BR>현재 유행하고 있는&nbsp;H3N2형이 국내에서 유행할 가능성도 높습니다.<BR><BR>다만 이번 독감의 바이러스 유형과 전파 양상으로 보면&#8230; 현재까진 2009년 전세계적인 <BR>신종플루 대유행과 같은 비상사태가 발생할 가능성은 현재로서는 별로 없어 보입니다.<BR><BR>계절성 독감의 피크 시기가 예년보다 조금 더 앞당겨 진 정도의 상황이라 판단되며, <BR>어린이와 노약자는 독감 예방주사를 접종받을 필요가 있고&#8230; 보건당국은 전 세계적인 상황을<BR>예의주시하면서 백신과 치료약을 충분히 비축해두고, 입원실 확보 등 신속 대응체계를 점검<BR>할 필요가 있을 것 같습니다.<BR><BR>그래도 해마다 겨울철 인플루엔자 바이러스로 인하여 수 많은 사람들이 사망하고 있으며,<BR>올해는 평균보다 사망자 수가 더 늘어날 가능성이 높다는 점을 유의해야 합니다.<BR><BR>================<BR><SPAN><BR>&nbsp;</P><br />

<H3>Synopsis:</H3><br />
<P>During week 52 (December 23-29), influenza activity increased in the U.S.</P><br />
<UL sizset="327" sizcache="3"><br />
<LI type=circle sizset="327" sizcache="3"><A href="http://www.cdc.gov/flu/weekly/#S1"><STRONG>Viral Surveillance:</STRONG></A> Of 9,363 specimens tested and reported by U.S. World Health Organization (WHO) and National Respiratory and Enteric Virus Surveillance System (NREVSS) collaborating laboratories, 2,961 (31.6%) were positive for influenza.<br />
<LI type=circle sizset="328" sizcache="3"><A href="http://www.cdc.gov/flu/weekly/#S2"><STRONG>Pneumonia and Influenza Mortality: </STRONG></A>The proportion of deaths attributed to pneumonia and influenza (P&#038;I) was below the epidemic threshold.<br />
<LI type=circle sizset="329" sizcache="3"><A href="http://www.cdc.gov/flu/weekly/#S3"><STRONG>Influenza-Associated Pediatric Deaths:</STRONG></A> Two influenza-associated pediatric deaths were reported and were associated with influenza B viruses.<br />
<LI type=circle sizset="330" sizcache="3"><A href="http://www.cdc.gov/flu/weekly/#S4"><STRONG>Outpatient Illness Surveillance:</STRONG></A> The proportion of outpatient visits for influenza-like illness (ILI) was 5.6%; above the national baseline of 2.2%. Nine of 10 regions reported ILI above region-specific baseline levels. New York City and 29 states experienced high ILI activity; 9 states experienced moderate ILI activity; 4 states experienced low ILI activity; 6 states experienced minimal ILI activity, and the District of Columbia and 2 states had insufficient data.<br />
<LI type=circle sizset="331" sizcache="3"><A href="http://www.cdc.gov/flu/weekly/#S5"><STRONG>Geographic Spread of Influenza:</STRONG></A> Forty-one states reported widespread geographic influenza activity; 7 states reported regional activity; the District of Columbia reported local activity; 1 state reported sporadic activity; Guam reported no influenza activity, and Puerto Rico, the U.S. Virgin Islands, and 1 state did not report. </LI></UL><br />

<P>Since the start of the season, influenza A (H3N2) viruses have predominated nationally, followed by influenza B viruses, while 2009 H1N1 viruses have been identified rarely. The predominant circulating virus has varied by state and by region.</P><BR><BR><br />
<H2>Antigenic Characterization:</H2><br />
<P>CDC has antigenically characterized 413 influenza viruses [17 2009 H1N1 viruses, 281 influenza A (H3N2) viruses, and 115 influenza B viruses] collected by U.S. laboratories since October 1, 2012.</P><br />
<P><STRONG>2009 H1N1 [17]:</STRONG></P><br />
<UL><br />
<LI>• All 17 2009 H1N1 viruses tested were characterized as A/California/7/2009-like, the influenza A (H1N1) component of the 2012-2013 influenza vaccine for the Northern Hemisphere. </LI></UL><br />
<P><STRONG>Influenza A (H3N2) [281]:</STRONG></P><br />
<UL><br />
<LI>279 (99.3%) of the 281 H3N2 influenza viruses tested have been characterized as A/Victoria/361/2011-like, the influenza A (H3N2) component of the 2012-2013 Northern Hemisphere influenza vaccine.<br />
<LI>2 (0.7%) of the 281 H3N2 viruses tested showed reduced titers with antiserum produced against A/Victoria/361/2011. </LI></UL><br />
<P><STRONG>Influenza B (B/Yamagata/16/88 and B/Victoria/02/87 lineages) [115]:</STRONG></P><br />
<UL><br />
<LI><STRONG>Yamagata Lineage [79]: </STRONG>79 (68.7%) of the 115 influenza B viruses tested so far this season have been characterized as B/Wisconsin/1/2010-like, the influenza B component of the 2012-2013 Northern Hemisphere influenza vaccine.<br />

<LI><STRONG>Victoria Lineage [36]: </STRONG>36 (31.3%) of 115 influenza B viruses tested have been from the B/Victoria lineage of viruses. <BR></LI></UL><br />
<H2 sizset="343" sizcache="3">Antiviral Resistance:</A><A name=S9 jQuery16209405785460762099="337"></A></H2><br />
<P>Testing of 2009 H1N1, influenza A (H3N2), and influenza B virus isolates for resistance to neuraminidase inhibitors (oseltamivir and zanamivir) is performed at CDC using a functional assay. Additional 2009 influenza A (H1N1) clinical samples are tested for a single mutation in the neuraminidase of the virus known to confer oseltamivir resistance (H275Y). The data summarized below combine the results of both testing methods. These samples are routinely obtained for surveillance purposes rather than for diagnostic testing of patients suspected to be infected with antiviral-resistant virus.</P><br />
<P>High levels of resistance to the adamantanes (amantadine and rimantadine) persist among 2009 influenza A (H1N1) and A (H3N2) viruses (the adamantanes are not effective against influenza B viruses). As a result, data from adamantane resistance testing are not presented below.</P><!--smarker3--><br />

<P><SPAN>The activity of seasonal influenza continues to rise in a number of European countries, especially in western Europe, concludes the latest </SPAN><SPAN><A href="http://www.ecdc.europa.eu/en/publications/Publications/Forms/ECDC_DispForm.aspx?ID=1028">Weekly Influenza Surveillance Overview, Week 52</A></SPAN><SPAN>. </SPAN><SPAN>There is still time to protect yourself from getting or spreading the flu by&nbsp;being vaccinated.</SPAN></P><br />
<P><SPAN></SPAN>&nbsp;</P><br />
<P><SPAN><SPAN><STRONG>Influenza season on the rise</STRONG></SPAN></SPAN></P><br />
<P><SPAN><SPAN></SPAN></SPAN><SPAN>Four countries (France, Italy, the Netherlands and Norway) have reported medium intensity influenza transmission; geographic spread of influenza activity was reported as widespread by five countries (Belgium, Denmark, France, Norway and the UK (England)); and nine countries have reported increasing trends. The proportion of sentinel specimens that are positive for influenza has exceeded 20% for two consecutive weeks. Countries undertaking surveillance of laboratory-confirmed severe influenza cases are starting to report increasing numbers of such individuals having to be hospitalised.</SPAN><SPAN> </SPAN></P><br />
<P><SPAN></SPAN><SPAN>So far the detected viruses remain well-matched to the vaccine strain and there has been no change in the viruses from<SPAN> <A href="http://www.ecdc.europa.eu/en/publications/Publications/Forms/ECDC_DispForm.aspx?ID=979">what was seen last season</A></SPAN>. Since week 40/2012, 46% of sentinel specimens were type A and 54% were type B, though the proportion of B viruses has decreased somewhat in recent weeks. Subtying of type A viruses has shown proportions of 56% A(H3) and 44% A(H1). </SPAN></P><br />
<P><SPAN></SPAN>&nbsp;</P><br />
<P><SPAN></SPAN><SPAN><SPAN><SPAN><SPAN><SPAN><STRONG>Influenza should be taken seriously</STRONG></SPAN></SPAN></SPAN></SPAN></SPAN></P><br />
<P><SPAN><SPAN>Influenza comes to Europe every winter – each year, around 10% of us are infected, and influenza-related complications cause hundreds of thousands of hospitalisations all across Europe. We can reduce the risk of getting influenza or transmitting it to others by a number of preventive measures, from applying better hand hygiene and not sneezing on others to staying at home when sick. <SPAN>Older people as well as those with chronic conditions suffer the most, but even perfectly healthy young people are at risk of developing serious complications, which can be fatal. Influenza-related deaths seen in recent weeks, for example, in Norway and Poland, have been reported in the media; sadly, they highlight what is unfortunately to be expected.</SPAN> Therefore people whose symptoms of the flu are worsening should seek medical advice as recommended by their national or local authorities.</SPAN></SPAN></P><br />
<P><SPAN><SPAN></SPAN></SPAN>&nbsp;</P><br />
<P><SPAN><SPAN><SPAN><STRONG>Still time to get protected</STRONG></SPAN></SPAN></P><br />
<P><SPAN></SPAN><SPAN>The single most effective way to protect yourself as well as those around you is by getting vaccinated. Influenza transmission has significantly progressed since Week 48 when ECDC Director <A href="http://www.ecdc.europa.eu/en/press/news/Lists/News/ECDC_DispForm.aspx?List=32e43ee8%2De230%2D4424%2Da783%2D85742124029a&#038;ID=805&#038;RootFolder=%2Fen%2Fpress%2Fnews%2FLists%2FNews">Marc Sprenger encouraged all Europeans</A> who are recommended to have the influenza vaccine by their national authorities to get vaccinated. It is still not too late to do so now. Influenza vaccination is of particular importance for those in the recognised risk groups and for healthcare workers, as it protects their vulnerable patients as well as themselves.</SPAN></P><br />
<P><SPAN>Throughout the influenza season, ECDC continues to monitor the situation and provide weekly surveillance overview and maps on seasonal influenza activity in Europe.<BR><BR>=========================<BR><BR></P><br />

The greatest pressures were in hospitals and especially critical or intensive care. Already this autumn the first affected European countries are seeing the same and a few European countries are known to have made plans to significantly increase their capacity, for example for ventilating patients. Reports on epidemiological and clinical presentation of influenza pandemic in other countries provide reliable estimates of the burden of disease, the effectiveness of treatment and best use of public health resources and likely outcomes for patients. Recently ECDC attended a WHO global consultation to gather such information. This Public Health development summarises some of the findings from that meeting and seven recent publications. <P>STOCKHOLM (Reuters) – The H1N1 <SPAN class=yshortcuts id=lw_1257519703_0>pandemic flu virus</SPAN> could kill up to 40,000 people across <SPAN class=yshortcuts id=lw_1257519703_1>Europe</SPAN> and be followed by seasonal flu waves that could kill the same number, European health experts said on Friday.</P><br />
<P>The <SPAN class=yshortcuts id=lw_1257519703_2>Sweden</SPAN>-based European Center for Disease Prevention and Control said epidemics of H1N1, known as swine flu, were now affecting almost all countries in the European Union but it could not predict how intense the peaks would be.</P><br />
<P>What was certain, it said, was that the <SPAN class=yshortcuts id=lw_1257519703_3 style="CURSOR: hand; BORDER-BOTTOM: #0066cc 1px dashed">pandemic</SPAN> would continue to kill thousands and put many patients into intensive care as the <SPAN class=yshortcuts id=lw_1257519703_4>northern hemisphere</SPAN>&#8216;s winter sets in.</P><br />
<P>&#8220;All <SPAN class=yshortcuts id=lw_1257519703_5>European countries</SPAN> will be affected, and this will put considerable stress on healthcare systems,&#8221; said ECDC director <SPAN class=yshortcuts id=lw_1257519703_6 style="CURSOR: hand; BORDER-BOTTOM: #0066cc 1px dashed">Zsuzsanna Jakab</SPAN>.</P><br />
<P>The ECDC, which monitors disease in the <SPAN class=yshortcuts id=lw_1257519703_7 style="BACKGROUND: none transparent scroll repeat 0% 0%; CURSOR: hand; BORDER-BOTTOM: medium none">European Union</SPAN> and European <SPAN class=yshortcuts id=lw_1257519703_8>free trade area</SPAN> (EFTA), said it was hard to predict what the mix of pandemic and seasonal flu viruses would bring but there was a risk of seasonal flu epidemics &#8220;early in 2010 when the pandemic waves have passed.&#8221;</P><br />
<P>Angus Nicoll, the ECDC&#8217;s flu coordinator, said in non-pandemic situations, seasonal flu could kill up to 40,000 <SPAN class=yshortcuts id=lw_1257519703_9>people in Europe</SPAN> &#8212; and H1N1 could do the same.</P><br />
<P>&#8220;That is not a trivial number,&#8221; he said. &#8220;And the fact that H1N1 is happening in younger adults, pregnant women and people without <SPAN class=yshortcuts id=lw_1257519703_10>risk factors</SPAN> &#8230; makes it feel different.&#8221;</P><br />
<P>RISKS AND DEATHS</P><br />
<P>The ECDC said experience from the United States and the <SPAN class=yshortcuts id=lw_1257519703_11>southern hemisphere</SPAN> showed pregnant women with the virus are 10 times more likely to need intensive care than those with no risk factors. Those with <SPAN class=yshortcuts id=lw_1257519703_12>asthma</SPAN> and <SPAN class=yshortcuts id=lw_1257519703_13>chronic respiratory diseases</SPAN> have three times the risk and the very obese six times the risk.</P><br />
<P>But it also said evidence so far shows <U>some 20 to 30 percent of H1N1 deaths are among healthy young people.</U></P><br />
<P>The ECDC&#8217;s latest daily update said all 27 EU and four EFTA countries have cases of H1N1 <SPAN class=yshortcuts id=lw_1257519703_14 style="CURSOR: hand; BORDER-BOTTOM: #0066cc 1px dashed">pandemic flu</SPAN> and there have been 389 deaths linked to H1N1 in the region since April, including 154 in <SPAN class=yshortcuts id=lw_1257519703_15>Britain</SPAN>, 73 in <SPAN class=yshortcuts id=lw_1257519703_16>Spain</SPAN>, 25 in Italy and 22 in France.</P><br />
<P>The ECDC&#8217;s global toll showed 6,005 fatal cases of H1N1 have been reported. The WHO, which updates its figures weekly, said on Thursday 5,712 people have died worldwide since H1N1 was discovered earlier this year.</P><br />
<P>The ECDC said the numbers of fatal cases associated with pandemic flu were &#8220;likely to be gross underestimates&#8221; as access to healthcare and lab tests varied from country to country.</P><br />
<P><SPAN class=yshortcuts id=lw_1257519703_17>GlaxoSmithKline</SPAN> and Sanofi-Aventis are among some 25 companies making <SPAN class=yshortcuts id=lw_1257519703_18>pandemic</SPAN> vaccines, while other drugmakers including Roche are making <SPAN class=yshortcuts id=lw_1257519703_19 style="BACKGROUND: none transparent scroll repeat 0% 0%; CURSOR: hand; BORDER-BOTTOM: medium none">antiviral therapies</SPAN> for use as frontline H1N1 treatment.</P><br />
<P>Nicoll said vaccination programs which started in some <SPAN class=yshortcuts id=lw_1257519703_20 style="CURSOR: hand; BORDER-BOTTOM: #0066cc 1px dashed">European countries</SPAN> in recent weeks were vital to protecting those most at risk but had come too late to halt the disease.</P><br />
<P>&#8220;We&#8217;re not trying at this stage to protect the whole of society with the vaccines. The strategy is to protect the vulnerable.&#8221;</P><br />

<P>A MAJOR medical monitoring group has claimed 80,000 Europeans could die from swine flu in the coming months, naming Ireland as one of the EU’s hotspots for the deadly pandemic. </P><br />
<P>Figures released by the European Centre for Disease Prevention and Control (ECDC) show that in recent weeks almost every country on the continent has seen a sharp rise in swine flu cases and hospitalisations. </P><br />
<P>Despite the widespread increase, the influential group singled out Ireland and Iceland as being worst affected by the pandemic and witnessing the highest intensity of swine flu diagnoses per head of population anywhere in the EU. </P><br />
<P>In the past week alone there have been 30,000 cases of swine flu in Ireland, with 14 deaths in the Republic and a further 10 fatalities in the North since the pandemic began. </P><br />
<P>As a result, the ECDC has placed Ireland in the &#8220;very high&#8221; risk category for further swine flu deaths — one of just two countries in the crisis situation. </P><br />
<P>According to the ECDC survey, Sweden is next-highest on the swine flu risk list, followed by Britain, both of whom are in the &#8220;moderate&#8221; category. </P><br />
<P>In the 27 EU countries and four European Free Trade Area (EFTA) nations there have been <U>389 swine flu-related deaths to date</U>, including <U>14 in Ireland, 154 in Britain, 73 in Spain, 25 in Italy and 22 in France. </U></P><br />

<P>&#8220;This is a national emergency as far as the health services are concerned,&#8221; Ms Harney admitted. </P><br />
<P>&#8220;The issues that confront us are no different than issues that confront other health ministries right around the world, particularly in the EU. But I think we have the capacity working together to be able to vaccinate the population as quickly as possible.&#8221;</P><br />
